Waiver
By electronically signing this Waiver, you indicate that you:
1. Wish to volunteer for the National Cherry Blossom Festival, Inc. ("NCBF") at the 2023 National Cherry Blossom Festival.
2. Certify that you are at least 18 years old (if you are under 18, use the minor waiver form below), physically fit and able to volunteer.
3. Will present to NCBF, when you first report to NCBF in person (“Report Date”), the card issued by the Centers for Disease Control (“CDC”) showing that you have been administered a minimum of two doses of COVID-19 vaccine (one dose for Johnson & Johnson) at least 14 days before the Report Date.
4. Will comply with all laws and regulations while volunteering, including but not limited to all COVID-19 rules, regulations, policies, laws and guidance from NCBF, the CDC and applicable state and local health departments.
5. Will,
AS A PARADE VOLUNTEER: a) assume all risks of personal injury, property loss or death that may occur related to your volunteering with NCBF; and b) waive, along with your heirs, executors and administrators, all current and future legal claims against NCBF, the District of Columbia, the National Park Service, EventsDC, JM Best Entertainment Inc., Under the Sun Productions Inc., WJLA-TV and each of their respective sponsors, officers, directors, employees, agents, representatives, successors, and assigns (collectively, “Presenters”).
OR
AS AN NCBF VOLUNTEER: a) follow NCBF rules and directions; b) not take any action that would negatively impact the Festival or NCBF's reputation; c) notify the NCBF volunteer supervisor 48 hours ahead of your appointed volunteer date(s) and time(s) (each a "Shift") if you are not able to perform at a Shift; d) arrange your own transportation to and from each Shift; e) arrive 15 minutes before the Shift in the NCBF-designated clothing; f) perform your duties in a professional manner; g) not smoke or drink alcohol on any Shift or when wearing NCBF-designated clothing; and h) check out with the NCBF volunteer manager at the end of each of your Shifts; g) assume all risk of personal injury, property loss or death that may occur related to your volunteering with NCBF; and h) waive, along with your heirs, executors and administrators, all current and future legal claims against NCBF, and its sponsors, officers, directors, employees, agents, representatives, successors, and assigns (collectively, “Presenters”).
6. Authorize any necessary medical treatment to you while volunteering with NCBF, at your cost.
7. Acknowledge that you will not be: a) compensated for your volunteer service; or b) covered by any Presenter’s insurance.
8. Indemnify and defend, together with your heirs, executors and administrators, the Presenters from all liabilities, claims, actions, damages, costs or expenses, including claims based on a Presenter's negligence, arising out of or connected with third party claims related to your volunteering with NCBF.
9. Allow NCBF and NCBF’s agents, without reservation, limitation or additional compensation, to record, in any media, and use, broadcast or produce derivative works from, in any media and for all time, your name, face, likeness, voice and appearance.
10. Intend to be legally bound by signing this Waiver on the date below.

2. Allow your Child to volunteer for the National Cherry Blossom Festival, Inc. ("NCBF") at the 2023 National Cherry Blossom Festival
3. Certify that your Child is physically fit and able to volunteer.
4. Will present to NCBF, when your Child first reports to NCBF in person (“Report Date”), the card issued by the Centers for Disease Control (“CDC”) showing that your Child has been administered a minimum of two doses of COVID-19 vaccine (one dose for Johnson & Johnson) at least 14 days before the Report Date. This requirement does not apply if your Child is under 12 years of age.
5. Will ensure that your Child complies with all laws and regulations while volunteering, including but not limited to all COVID-19 rules, regulations, policies, laws and guidance from NCBF, the CDC and applicable state and local health departments.
6. Will:
IF YOUR CHILD IS A PARADE VOLUNTEER: a) assume all risks of personal injury, property loss or death that may occur related to your Child’s volunteering with NCBF; and b) waive, along with your Child and your respective heirs, executors and administrators, all current and future legal claims against NCBF, the District of Columbia, the National Park Service, EventsDC, JM Best Entertainment Inc., Under the Sun Productions Inc., WJLA-TV and each of their respective sponsors, officers, directors, employees, agents, representatives, successors and assigns (collectively, “Presenters”).
OR
IF YOUR CHILD IS AN NCBF VOLUNTEER: a) ensure that your Child follows NCBF rules and directions and does not to take any action that would negatively impact the Festival or NCBF's reputation; b) notify your Child's NCBF volunteer supervisor 48 hours ahead of my child's appointed volunteer date(s) and time(s) (each a "Shift") if your Child is not able to perform at a Shift; c) arrange for your Child's transportation to and from each Shift; d) ensure that your Child arrives 15 minutes before the Shift in the NCBF-designated clothing; e) ensure that your Child does not smoke or drink alcohol on any Shift or when wearing NCBF-designated clothing, f) ensure that your Child checks out with their NCBF volunteer manager at the end of each Shift; g) assume all risks of personal injury, property loss or death that may occur related to your Child’s volunteering with NCBF; and h) waive, along with your Child and your respective heirs, executors and administrators, all current and future legal claims against NCBF, and its sponsors, officers, directors, employees, agents, representatives, successors and assigns (collectively, “Presenters”).
7. Authorize any necessary medical treatment to your Child while volunteering with NCBF, at my cost.
8. Acknowledge that your Child will not be: a) compensated for his or her volunteer service; or b) covered by any Presenter insurance.
9. Indemnify and defend, together with your Child and your respective heirs, executors and administrators, the Presenters from all liabilities, claims, actions, damages, costs or expenses, arising out of or connected with third party claims related to your Child's volunteering with NCBF.
10. Allow NCBF and NCBF's agents, without reservation, limitation or additional compensation, the right to record, in any media, and to use, broadcast or produce derivative works from, in any media and for all time, your Child’s name, face, likeness, voice and appearance.
11. Intend to be legally bound by signing this Waiver on the date below.
